Geriatrics

1. Philadelphia Corporation for the Aging (215-765-9040)- eligibility: age 60 or 18 y.o. w/ disability- provides home care services (laundry/groceries), not home nursing

2. LIFE program- Same as above occors all within the LIFE building.  Must relinquish role as primary care provider for participation in program (PCA will evaluate for this program if asked)

Weight Loss

1. Center for Weight and Eating Disorders (215-898-7314)- Counselling on weight loss-- for as little as $5 co-pay

2. Stunkard Weight Mangement Program (215-746-4100)- Behavioral Modification Therapy & Meal Replacement Program-- not taking insurance at present, about $600

3. Bariatric Surgery Program UPHS (215-615-7500)
