Presbyterian ER Rotation
Location:
1st floor, Presbyterian Hospital
 
Setup of Rotation:
This is a great month of one-to-one attending-intern teaching, opportunity to do procedures and actually have decent work hours! You work shifts, with 2wks of night shift (9pm to 6am) and 2wks of day shift (9am to 7pm). You usually have 2 weekends completely off. Your PFC continuity clinic is every Friday morning.
The ER runs on the EMTRAC computer system that is on every computer in the ER. You will be given a brief orientation on the system on your first day. The system shows which patient is in what room and what their chief complaint is. It also shows how long they have been waiting and who their nurse is. Find a patient that is not yet assigned to an attending or resident, and assign yourself to that patient (they will show you how). Then grab their chart from the triage rack and go and see the patient, doing an appropriate history and physical. Then find an attending to present your patient to, and discuss the patient and orders. You should then put in the appropriate orders in EMTRAC, write up the patient in EMTRAC, and follow up on results as they come back. In the mean time you should start seeing another patient. Keep track of your prior patients as they are your responsibility to discharge or admit as you see fit and as discussed with your attending. You will be responsible for “calling up” or giving report to the admitting resident when your patients are admitted. There are usually1-2 attendings in the ER at any time, and anywhere from 1-5 residents/students. Each resident/student sees patients individually and presents directly to one of the attendings. Each room has a bedside pixus machine for supplies, the code is 8214 (=ER phone number).
 
 
Computer Systems:
EMTRAC: You need to get your username and password for this system preferably before the rotation starts. Ask Darcy how to set this up. All ER information is in the EMTRAC system. This includes all patient information, prior visits, triage notes, nursing/resident/attending notes, orders and lab results.
 
Medview: this is used to see more extensive records on a patient, including past discharge summaries/labs/radiology studies.
 
Presby-cards: Here is where you can access all cardiology reports on a patient including echo, EKG, cath lab reports. Website is http://pmc-card.uphs.upenn.edu
 
Geocities.com/pennmedicine: this is a link where you can find pager numbers on EVERYONE in the internal medicine department.
 
Magicweb: this is where all the radiology images are.   This system automatically interfaces with medview.
 
Charts:
Triage charts are in the appropriately numbered bin for the patient-these charts will sometimes contain notes from an outside facility or other documents. However all notes/vitals etc are in the computer system (EMTRAC)
Parking:
1. Park in the Powelton/39th Street lot during day shifts
2. For night shifts, you may park in the covered Presbyterian lot as long as you arrive after 2pm and exit by 9pm.     Again, use your presby ID to get in and out.
 
Cafeteria:
No meal tickets for this rotation. Bring your own lunch or go to the cafeteria or an outside food truck. You can eat in the lounge in the ER for which the code is 2 and 1 together then 5.
